Reservation Request Form

Please note that we require at least a 2 week notice for ALL group visits.

Please fill out this form completely to request a reservation. If we are unable to accommodate your request a representative from the Education Department will contact you to discuss alternate arrangements. Otherwise, a Tentative Reservation will be made based on the information that you have provided. Once your requestd event has been scheduled a copy of your Tentative Reservation/Confirmation Sheet will be sent to you via e-mail. If you do not receive the Tentative Reservation/Confirmation Sheet within 4 business days, please contact the Education Department at (508) 853-6015, ext. 20.
